Access, Navigation and Coordination of Home Care Services
Access, navigation and coordination of home care and personal support services is a common role for many caregivers. To support our work to inform health system leaders about the caregiving experience, OCO would like to better understand of this part of the caregiver experience.
We are seeking caregivers who are willing to share their experience in finding and coordinating services to support the daily activities of their care recipient. We are hoping to hear from people whose caregiving experience includes one or more of the following:
Home care services accessed through a care coordinator (you may know this as Home and Community Care Support Services, LHIN, CCAC, personal support worker (PSW) services or respite)
Services accessed through programs of the Ministry of Children, Community and Social Services (e.g. Personal support worker or respite services accessed through programs of the Ministry of Children, Community and Social Services including Developmental Services Ontario (DSO) passport funding).
Government-led programs that provide a budget for families to hire and coordinate their own services (these may be called self-directed, family-managed home care, special services at home).
Private hire of home care or personal support services
If you are interested in providing feedback you can participate by completing the survey (see below) and/ or participating in a focus group scheduled for Monday April 3rd , 5-6pm. We will contact individuals about the focus group by Friday March 31st, 2023.
